4. Detailed proposal including project objectives, budget, committed and prospective funding sources, timetable for implementation, target population, and expected outcomes. Please document all capital expenses.
5. Evaluation plan and prospect for continued support of the project.
6. Resume of project officer and a list of key staff.
7. Copy of the Internal Revenue Service section 501(c)(3) determination letter granting tax-exempt status.

You will receive an acknowledgement of the receipt of a grant proposal. While the grant request is being reviewed, Foundation trustees or staff may request a telephone conference, site visit or presentation. Please do not visit the Foundation offices or contact our staff during the grant proposal review process.

Initial Inquiry
Prior to submitting a grant request, each prospective applicant must submit a brief letter to the Executive Director, which should include: Contact information, background of the organization, a brief project description, and specific funding amount being requested.
Letters of inquiry will be accepted between June 1 and September 1.

The Foundation will review all inquiries and provide a written response, either with an explanation for the denial, a request for further information, or an invitation to submit a full grant proposal. We ask that applicants only submit a detailed grant proposal after they have received
a favorable response to their initial inquiry.

Grant Proposal Requirements
When invited to submit a grant request, the request will be accepted during the period beginning September 1 and ending October 31.
If sent by mail, it must be postmarked no later than October 31. If hand delivered, it must be in our office by 2 P.M. on the last business day before October 31.Complete grant proposals will be reviewed, and grants will be awarded by May 1 of the following year.
All requests must be made in writing and include:

1. Completed Proposal Cover Sheet
2. Executive summary with brief statement
of the project, its goals, financial requirements and status of the project. You must request a
specific grant amount between $5,000 and $50,000.
3. Brief history of the organization, an annual report, two years of the most recent financial statements (preferably audited), and the names and occupations of all directors and trustees.
